BSA National ABC medical form

This form is required by all participants, youth, and adults. It must be filled out completely. Parts A, B, and C, to include a Doctor's signature.

Please ensure you complete all parts, Attached Immunizations records will not be accepted, please fill out the form for your safety.

A Medical form is only valid for 12 months, Please ensure your physical was signed after 23 December 2016 to be valid

NOTE: All forms are to be filled out so they are legible. Fill them in, print, sign, scan, and email to "NYLT@fareastcouncil.org" or to "fec.nylt.course.director@gmail.com" or upload here.
All applications required paperwork (physical, YPT), and full payment must be submitted no later than 1 June 2018.
Important:
Medical Form C is the annual physical required by National. These are due as soon as possible with a deadline of June 1, 2018. If your Medical Form C is not received by June 1, 2018, the applicant will be placed on a waitlist. Your space will not be reserved. All confirmations are based on complete forms.
